Diffuse Lichen Planus in a 6-Year-Old Female

Abstract
Pediatric lichen planus (LP) is rare and may be triggered by vaccines through autoimmune mechanisms such as molecular mimicry. We present a case involving a 6-year-old girl who presented with pruritic, violaceous papules and hyperpigmented patches consistent with LP, which developed two months after MMR, varicella, and TDap vaccination. Initial treatment with high-potency topical corticosteroids failed, prompting initiation of a systemic prednisolone taper, which led to marked clinical improvement. This case contributes to the limited pediatric literature of diffuse LP, emphasizing the importance of early recognition and individualized management of widespread disease.
